Your Personal Information.
The personal information members provide us is collected solely for the purpose of providing members and their dependents (if applicable) with the National Benefit Plans services and benefits. This information is protected by National Benefit Plans by maintaining physical, electronic and procedural safeguards.
Personal information we collect about you comes from one or more of the following sources: paper application, faxed application, and web application. Personal information collected includes, but is not limited to: name, address, city, state, zip, telephone numbers, date of birth, gender, dependent name, dependent gender, dependent date of birth, dependent relationship, beneficiary name, birth, gender, bank account information, or credit card information.
We may disclose any or all of this information based on business need only, to our affiliates, service providers, third party service providers, provider networks and other suppliers of products and services to National Benefit Plans.
National Benefit Plans will maintain member information even after membership is terminated for any reason. This information will be protected in the same manner as that of an active member. A member may request in writing at any time, after discontinuing the membership to remove the information from our file.
Sharing Your Personal Information with National Benefit Plans affiliates and Service Providers.
We will share member information with its National Benefit Plans affiliates, administrator, related companies and service providers to the extent it is necessary to provide and manage benefits for the member or their dependents. Unless otherwise required by law, National Benefit Plans will not share with these parties any information that it collected that is not needed by these parties to provide the benefits to the member or their dependents. These organizations are not authorized to use member information for any purposes other than for National Benefit Plans business.
Sharing information with Third Parties or Service Providers not affiliated with National Benefit Plans.
We will not reveal member information to any external organizations (other than affiliates and service providers mentioned above), unless the member specifically authorizes us to do so or as required by law.
National Benefit Plans Commitment to Members.
Keeping our member information secure and using it only as needed for business reasons to provide services to our members is a top priority to National Benefit Plans. We will safeguard, according to strict standards of security and confidentiality, any information provided to us by our members. We will permit only authorized employees who are trained in the proper handling of member information to have access to that information. Employees who violate our Privacy Policies will be subject to our normal disciplinary process.
